Case Management Services

Bayview House

A medium support, coed group home in North York with a capacity of ten people. Residential Counselors are available weekdays in the afternoon and evening, and on afternoons on most weekends. Residents must be able to live in a group setting with this amount of support. Residents share bedrooms.

Bayview Co-op

A low support, coed group home in North York with a capacity of six people. A Residential Counsellor is available two evenings per week. Residents must be motivated toward peer support and want to live in a group setting. Residents have their own bedrooms.

Case Management Services

Our flexible support program provides intensive Case Management services in northeast Toronto. This program is intended for individuals who have, or are at risk of having, a serious mental illness and both need and want intensive supports to live satisfactorily and successfully in the community. Our clients have access to one and two-bedroom apartments, when available, through our partnership with T.C.H.C.. Many of these clients are referred directly by one of our partner Hospitals, The North York Hospital or Sunnybrook Health Sciences Centre.

Referrals

Referrals to Bayview Community Services originate from mental health and social services agencies, psychiatrists, hospitals, and other housing programs. Enquiries from individuals or families are welcome, though a professional referral is necessary.

Referral information packages for each program are available on request, when waiting lists are open. Completed referral forms for our Group Homes should be returned to our Intake Coordinator. Completed Access 1 referral forms for Case Management services are processed through the Access 1 office.

Most applicants to our programs will be interviewed.

Decisions on acceptance to our programs are made by our staff team and are based on the determined benefit of the program for the applicant. Please contact our Intake Coordinator prior to referring a client.
